当院では、医療の透明化や患者様への情報提供を積極的に推進していく観点から、領収書の発行の際に個別の診療報酬の算定項目のわかる明細書を無料で発行しております。

 

• 一般名処方加算について

当院では、後発医薬品の使用促進を図るとともに、医薬品の安定供給に向けた取り組みなどを実施しています。昨今、一部の医薬品について十分な供給が難しい状況が続いています。後発医薬品のある医薬品について、特定の医薬品名を指定するのではなく、薬剤の成分をもとにした一般名処方を行う場合があります。一般名処方によって特定の医薬品の供給が不足した場合であっても、患者様に必要な医薬品が提供しやすくなります。

※一般名処方とは、お薬の「商品名」ではなく「有効成分」を記載して処方することです。

 

• 電子的診療情報連携体制整備加算について

1. オンライン請求を行っています。

2. オンライン資格確認を行う体制を有しています。

3. オンライン資格確認システムを通じて得た情報(受診歴、薬剤情報、特定検診情報、その他診療情報)を医師が診察室で閲覧、活用できる体制を有しています。

4. マイナンバーカードの健康保険証利用を促進し、医療DXを通じて質の高い医療を提供できるよう取り組んでいます。

5. 電子処方箋を発行できる体制を有しています。

6. 電子カルテ情報共有サービスを活用できる体制については、今後導入する予定としています。

 

• 外来感染対策向上加算について

1. 当院は自治体の要請に応じ、新興感染症の発生時に発熱患者の診察を行う 「第二種協定指定医療機関」 として指定されています。

2. 当院では受診歴の有無に関わらず、発熱およびその他の感染症を疑わせるような症状を呈する患者様の受け入れを行っています。

3. 感染管理者である院長を中心に、全職員で院内感染対策を徹底しています。

4. 職員が院内感染防止の基本理念と知識を深めるため、年２回の研修会を実施しています。

5. 抗菌薬の使用に際しては、厚生労働省のガイドラインに基づき適切に対応します。

6. 標準的感染予防策に基づくマニュアルを作成し、全職員がそれに沿って院内感染対策を推進していきます。

7. 感染対策に関して基幹病院と連携体制を構築し、定期的に必要な情報やアドバイスを得て院内感染対策の向上に努めています。

 

• 長期処方・リフィル処方箋について

当院では患者さんの状態に応じて、２８日以上の長期の投薬あるいはリフィル処方箋を発行することができます。

※なお、長期処方やリフィル処方箋の交付が対応可能かは、患者さんの病状に応じて担当医が判断いたします。

 

• 生活習慣病管理料Ⅱ （高血圧症・脂質異常症・糖尿病） について

医師が、高血圧症・脂質異常症・糖尿病を主病として治療している患者さんに、個々に応じた目標設定や血圧、食事、運動に関する具体的な指導内容、検査結果を記載した 「療養計画書」 を作成し、より実効性のある疾患管理を行ってまいります。

また、患者様の状態に応じ医師の判断のもと、２８日以上の長期の投薬を行うことがあります。

◆ Regarding the Surcharge for Issuing Detailed Statements
At our hospital, in order to actively promote transparency in medical care and provide information to patients, we issue detailed statements showing the individual medical fee calculation items free of charge when issuing receipts.

◆ Regarding the Surcharge for Generic Prescriptions
At our hospital, we are working to promote the use of generic drugs and to ensure a stable supply of medicines. Recently, there has been a continuing difficulty in adequately supplying some medicines. For medicines for which generic versions exist, we may prescribe using the generic name based on the drug's active ingredient, rather than specifying a particular drug name. Generic prescribing makes it easier to provide patients with the necessary medicines even if there is a shortage of a specific medicine.
*Generic prescribing means prescribing by listing the "active ingredient" of the medicine, rather than the "brand name."

 

◆ Regarding the Surcharge for Establishing an Electronic Medical Information Collaboration System
1. We conduct online billing.
2. We have a system in place for online eligibility verification.
3. We have a system in place that allows doctors to view and utilize information obtained through the online eligibility verification system (medical history, medication information, specific health checkup information, and other medical information) in the examination room.

4. We are working to promote the use of My Number Cards as health insurance cards and to provide high-quality medical care through medical DX.

5. We have a system in place to issue electronic prescriptions.

6. We plan to introduce a system that allows the use of electronic medical record information sharing services in the future.

 

◆ Regarding the Outpatient Infection Control Improvement Surcharge
1. Our hospital has been designated as a "Type 2 Agreement Designated Medical Institution" in response to requests from local governments to examine patients with fever in the event of an outbreak of an emerging infectious disease.

2. Our hospital accepts patients exhibiting symptoms suggestive of fever or other infectious diseases, regardless of whether they have a history of visits.
To separate the flow of general patients from patients with fever, please be sure to make a telephone reservation before your visit.

3. Under the leadership of the hospital director, who is the infection control manager, all staff members are thoroughly implementing infection control measures within the hospital.

4. We conduct training sessions twice a year to deepen staff members' understanding of the fundamental principles and knowledge of infection control within the hospital.

5. When using antibiotics, we respond appropriately based on the guidelines of the Ministry of Health, Labour and Welfare.

6. We have created a manual based on standard infection prevention measures, and all staff members promote infection control in accordance with it.

7. We have established a collaborative system with core hospitals regarding infection control, and we strive to improve our infection control measures by regularly obtaining necessary information and advice.

 

◆ Regarding Long-Term Prescriptions and Refill Prescriptions
At our hospital, we can issue long-term prescriptions of 28 days or more, or refill prescriptions, depending on the patient's condition.

*Please note that the attending physician will determine whether long-term prescriptions or refill prescriptions are possible based on the patient's medical condition.

 

◆ About Lifestyle-Related Disease Management Fee II (Hypertension, Dyslipidemia, Diabetes)
For patients whose primary condition is hypertension, dyslipidemia, or diabetes, physicians will create a "Treatment Plan" that includes individualized goals, specific guidance regarding blood pressure, diet, and exercise, and test results, thereby enabling more effective disease management.

Furthermore, depending on the patient's condition, long-term medication for 28 days or more may be prescribed at the physician's discretion.

Our hospital complies with the following facility standards set by the Minister of Health, Labour and Welfare and has reported to the Kanto-Shinetsu Regional Bureau of Health and Welfare before providing medical care.
